Either after a dry spell of mediocre lp's, or re-grouping...
Black Sabbath - Heaven and Hell (Wow, who saw that coming after NSD?!)
Deep Purple - Perfect Strangers (Probably my fave from them.)
Saxon - Lionheart (Definitely my fave from them!)
Judas Priest - Angel of Retro (The return of the Metal God!)
Stryper - No More Hell to Pay (I was blown away!)
Saint - Hell Blade (Didn't think they had this in them!)

Satan's Host - By The Hands of the Devil. The band never went away, but this was the return of the mighty Tyrant.

IMHO, none of the parameters you listed apply to this album and what came before it.
Unleash The Beast (97) Metalhead (99) Killing Ground (2001)
They are far from mediocre and had the same line up.

Overkill - Ironbound - the band came roaring back from a three year absence with an album that was a total ripper. Â Not that Overkill was ever terrible, but this album was ferocious.
Accept - Blood Of The Nations - Seemingly out of nowhere, the band comes back with a new lead singer that fits the band like a glove and a cracking album to boot.

Not a big Saxon fan, but I took a chance on this one based on the reviews at BWBK at the time of it's release, I was pleasantly surprised!

Van Halen - Different Kind of Truth. Not a perfect album but the majority of it is just what DLR-era fans have been hungry for since 1984.

Dokken..Lightning Strikes Again (what a great return to their 80s roots)
Motley Crue...Dr. Feelgood (after a few dry-spell albums comes this one..an album that even Metallica called a fantastic heavy mix)
Ace Frehley...Space Invader (The space Ace roars back showing that he is better than Kiss)
Ratt...Infestation (possibly the best album they ever put out)

I agree. Lionheart is awesome and one of their best, but those three you mentioned are all real good albums, too. Especially Unleash the Beast. I think that might count as a comeback album.

As for re-groupings, I should nominate AC/DC's Back in Black.
All 4 of those are awesome albums.
