I hate it when great metal albums actually have a non-metal cover.
Great covers are a must.

Assuming you are talking about the album covers themselves, here are a few that come to mind: Fates Warning - No Exit Raven - Wiped Out Black Sabbath - Born Again Mortification - Brain Cleaner

Black Sabbath - Born Again is one ugly album cover. I never liked it.

Isn't it a matter of opinion what exactly is a "metal cover". I mean their are many great albums whose covers don't jump out and say "METAL ". There are also covers that LOOK like they would be metal albums but aren't.
Bat Out Of Hell is a perfect example of that.

The rap group Cypress Hill also have some very metal looking album covers, especially Black Sunday.
